Flexible
Case Management
We
offer Flexible Case Management though our affiliated agency
Pinnacle Services. Flexible Case Management is a service that
can be purchased through an individual's budget.
Our Flexible Case Managers are experienced professionals who
work collaboratively with families, individuals with disabilities,
County and State officials, and other community agencies to
carry out the duties that are required when using the Consumer
Directed Community Supports (CDCS) option.
We offer families and consumers support in a variety of areas.
Areas of support include, but are not limited to:
• Developing the Community Support Plan (CSP)
• Developing the Health and Safety Plan or Risk Management
Plan
• Overall management of funds
• Identifying and accessing community services and supports
• Securing employees
• Monitoring the CDCS plan and budget, and revising it
as needed. |
