Consumer Support Grant (CSG) and Family Support Grant (FSG) allow consumers the ability to purchase goods and services in the way that makes the most sense for them.
Individuals eligible to utilize Consumer Support Grant option include recipients of Medical Assistance, those who are eligible to receive home care services from the Medical Assistance Home Care Program, demonstrate limitations in everyday functioning, and require ongoing supports to live in the community and live in a natural home setting. Family Support Grant recipients include families of children with a certified disability, under age 21, living in their biological or adoptive home.
Consumer Support Grant enrollees may chose to purchase a variety of supports. These supports are identified in an approved Consumer Support Grant plan which is created by the consumer or consumer’s representative. The plan might include companionship services, respite care, home or vehicle modifications, nutrition services, specialized equipment and transportation.
Family Support Grant enrollees may chose to purchase supports such as computers, day care, educational services, medical services, medications, respite care, specialized clothing, specialized diet and/or equipment and transportation.
Summit Fiscal Agency will bill as the enrolled Medical Assistance provider, and verify documentation of allowable expenditures. We can provide technical assistance regarding employee and budget issues. Our employees enthusiastically work with families and case managers to analyze and support utilization within resource allocations. Summit Fiscal Agency may provide tracking, monitoring and reporting consumer use of the grant allocation.
Summit Fiscal Agency offers a choice of self-directed service models to individuals who choose to use the Consumer Support Grant or Family Support Grant including Fiscal Support Entity, Agency with Choice, Fiscal Conduit and Payroll Agent. As an experienced FSE, Summit Fiscal Agency is also a seasoned Personal Support, Homemaker, and Unlicensed Respite provider for most waiver eligible individuals.
